One of the local eye hospitals has brought to our attention the fact that some child patients are being denied repair/replacement GOS4 vouchers when they have originally been issued with an HES(P) from the hospital.
We are asked to remind you that GOS4 forms CAN be used to repair/replace spectacles which have been prescribed on an HES(P) and parents should not be told to go back to the hospital for a new voucher to effect such a repair/replacement.
