The ICB would like to remind practices that they are not endorsing any provider specific referral forms across the Fylde Coast and Central Lancs. All patients will be given a choice discussion when the referral is received at the RMC. It causes confusion if the referral is the provider form as the patient may choose a different provider. Please could we ask that these are taken out of circulation. Best practice is to send cataract referrals via Opera, if having to send via email please use one of the following forms
