The policy provides guidance which is intended to assist contractors and L&SC ICB in processing requests for second pairs and to support consistent decision making. Please ensure you and your staff familiarise yourselves with the Decision-Making Policy and use the below new Second Pairs application form for requests as from today. The ICB have advised practices to include as much information as possible to support the application.
