The Senior Primary Care Manager for NHSEI/NWest has asked the LOC to send the following form out to all those who wish to have a Covid jab and who have not already had one or made a booking.
Please complete this form, then email it to Paul Hutchence at lmblocsec@gmail.com He shall then submit the list when requested to the PCNs and Trusts who will make arrangements for you to book an appointment.
